Título

Potentially inappropriate ICU admissions of cancer patients: assessing differences in perceptions between intensivists and oncologists

Introdução

Identifying oncology patients with a low likelihood of benefiting from ICU admission is crucial to avoiding unnecessary interventions. However, there are no specific guidelines for screening these patients, which can lead to conflicts in decision-making between intensivists and oncologists.

Objetivo

Evaluate the consensus between intensivists and oncologists on ICU admission triage for oncology patients.

Métodos

An anonymous, self-administered questionnaire was distributed among intensivists, oncologists, and hematologists. Participants evaluated real clinical cases, categorizing patients according to the Society of Critical Care Medicine criteria. The agreement between assessments was calculated using Fleiss' kappa coefficient. Associations between the physicians' characteristics and their triage decisions were examined. Categorical variables were analyzed using the chi-square test, while continuous variables were evaluated using ANOVA or the Kruskal-Wallis test, as appropriate.

Resultados

A total of 357 professionals were invited, and 97 completed the survey. Among the respondents, 90 (92%) were intensivists, while the remainder were hematologists or oncologists. Of these, 53 (53%) were women, and 51 (50%) were aged between 31 and 40 years. Furthermore, 80 (81%) had received some level of undergraduate training, such as lectures or seminars, on palliative care.

Conclusões

The results of this descriptive analysis highlight a predominance of intensivists among the participants, with significant female representation and extensive training in palliative care. This suggests a solid knowledge base in the field.
