COVERAGE OF THE NATIONAL CERVICAL CANCER SCREENING PROGRAM IN A RURAL AREA OF AMAZON, BRAZIL

Introdução

Worldwide, cervical cancer is the 4th most frequently diagnosed cancer and the 4th leading cause of cancer death in women. According to the WHO, screening coverage of at least 70% of the target population, guaranteeing adequate diagnosis and treatment of altered cases, it is possible to reduce, on average, from 60 to 90% the incidence of cervical cancer.

Objetivo

To analyze the coverage of cervical cancer screening among women in a rural area of the Amazon.

Métodos

It was a descriptive cross-sectional study with 304 women between 25 and 64 years old, who live in the rural community of Pau Rosa located in the rural area of the municipality of Manaus – Amazonas, Brazil (about 40 Km far form Manaus). We used SISCOLO and SISCAN to get data (Brazilian Cervical Cancer Information System) and data from the public Rural Health District of the from the Municipality of Manaus. We used the cytopathological examination records of the last 5 years (2018-2022). Descriptive data analysis was performed using EpiInfo version 7.2.5.0 for Windows. The study was approved by Institutional Review Board of the Fundação Centro de Controle de Oncologia do Estado do Amazonas (N. 2,802,179).

Resultados

A total of 304 participants were included in this study, with a mean age of 44.26 years. The screening coverage was 67.43% (205/304). Among the participants, 23.68% (72/304) underwent 1 cytopathological examination in the Health Unic System (SUS), followed by 16.77% (51/304) who underwent 2 cytopathological examinations, 15.78% (48/304) underwent 3 cytopathological tests, 7.89% (24/304) underwent 4 cytopathological tests, 3.28% (10/304) performed between 5 or 6 cytopathological tests and 32.56% (99/304) did not perform any cytopathological tests in this period. Between 2018 and 2022, 205 participants underwent a total of 467 cytopathological tests, of which 88.3% (181/205) had a negative result for neoplasia, 4.4% (09/205) had at least one of the results with an unsatisfactory sample, 0.5% (01/205) had at least one LSIL result, 4.8% (10/205) had at least one of the ASC-US results and 2% (4/205) had ASC-H or more.

Conclusões

We concluded that the coverage of the national cervical cancer screening program in the Pau Rosa community was 67.43%% between 2018 and 2022. Taking into account that it is below the target followed by the World Health Organization for the control of the disease, it is necessary to offer alternatives to overcome the access difficulties that women in this locality have to face to guarantee screening.
